MWAS Athlete Performance Feedback form

The Mid West Academy of Sport provides an elite home training environment for talented Mid West athletes. The athletes are from over a dozen different sports and all perform at different levels of competition from talent squads to Australian Teams at World Championship level. Some of the services we provide to these athletes are strength and conditioning training, sports psychology, behavioural profiles and workshops, physiotherapy, nutrition and on and off field development opportunities. In order for us to track the development of these athletes we require feedback on their performances at training and during events such as games and championships from their sports and their coaches. This what this survey is for and thank you for taking the time to complete it. (your answers will be used to plot their progress with them to continue their overall development and may not be anonymous).
  • Are you the High Performance Manager, Head Coach, Assistant Coach or other role?
  • Skill is an athlete's ability to choose and perform the right techniques at the right time, successfully, regularly and with a minimum of effort. Please rate the athletes use of their skills to achieve athletic objectives.
  • Dot points are fine and please provide examples of things that they can physically improve on and how.
  • The need for different aspects of physical preparation, including strength training, metabolic conditioning, and neuromuscular training has been described for a range of sports and for young athletes at different stages of maturation. Were they prepared appropriately for their sport?
  • Tactics are the skills required in any game that allows a player or team to effectively use their talent and skill to the best possible advantage. Tactics are plans which are set up for a specific purpose during a performance or match. Did they perform the tactics effectively and is their understanding of the game / sport sufficient?
  • 'Motivation' is a desire and determination to improve sports performances. To become the best athlete you can be, you must be motivated to do what it takes to maximise your ability and achieve your goals. Please evaluate how motivated the athlete is?
  • "Mental toughness" is the ability to resist, manage and overcome doubts, worries, concerns and circumstances that prevent you from succeeding, or excelling at a task or objective you set out to achieve. Describe how mentally tough the athlete is?
  • A short response is o.k or dot points.
  • This is at the championship or at the highest level competition they played in with you.
